Usually docked, with multiple terminals along the Tagus. Which terminal your ship uses matters: eastern terminals (Jardim do Tabaco, Santa Apolonia) are near Alfama; western terminals (Alcantara) are near Belem.
How to Get Into Town
Alfama is walkable from the eastern terminals (Jardim do Tabaco / Santa Apolonia). Belem requires a 7-minute train or 30-minute tram from Cais do Sodre. Sintra is 40 minutes by train from Rossio station.
Main Attractions
Belem holds Lisbon's most iconic monuments. Alfama is the city's oldest and most atmospheric neighborhood. Sintra is one of Portugal's UNESCO highlights and a full half-day on its own.
How Much Time You Need
A full 8-9 hour call covers either Belem or Alfama well. Sintra needs 10+ hours and a clear plan.
Keep in Mind
Tram 28 is the single most documented pickpocket location in Lisbon. Keep bags zipped and on your front, particularly at stops where passengers board in groups.
Safety & Comfort
Lisbon is safe and very well set up for tourists. Pickpocket awareness on Tram 28 and in crowded market areas is the main practical concern.

We spent the whole morning just walking around Alfama. Found a tiny tiled restaurant for lunch in an alley we never would have found with a guide. That said, Christine's knees did not enjoy the hills. We should have taken the tram up and walked down.
Tried to do Sintra AND Belem. Big mistake. We rushed through Sintra, rushed through Belem, were stressed the whole time, and got back to the ship with 20 minutes to spare. Pick one. Just pick one.
Went straight to Belem by train, had the monastery almost to myself at 10am, then walked to the pastry shop and joined the queue. It was long but moved fast. Those tarts are worth every minute of the wait.
