The afternoon tender queue is the real risk. Combined with the bus back down from Taormina, it eats more time than people expect, so head back early.

Taormina is beautiful but my word it's steep. The theatre climb finished us off a bit. If your legs aren't great, know that Giardini Naxos down by the ship is flat and lovely too, we wished we'd allowed more time just there.
Tender at 9, walked to the bus stop, 9:40 Interbus up, theatre by 10:15 before the crowds. Back on the 1pm bus and tendered by 2. That order worked perfectly. The bus stop is a bit of a walk from the pier so don't dawdle.
We tried to squeeze Etna in and it was too much with the tender, ended up rushing and stressed. Should have just done Taormina and Isola Bella. Kids loved the little island and the pebble beach in the end.
Missed two tenders because the queue was massive and nearly had a heart attack about the time. Taormina is gorgeous but nobody warns you how the afternoon tender crush works. Go back earlier than you think you need to.
