We went straight to the Neptune's Grotto ticket office the moment we stepped off the tender. Had our boat tickets within five minutes. The fellow at the desk said the 9:30am boat had two spots left. If we had gone for a coffee first, we would have missed it. Get the tickets first, explore the town after.
My daughter wanted to do the 654 steps. I said the boat. We did the boat. She was annoyed for about ten minutes and then completely forgot about it once she was inside the cave. The boat is easy. The cave is spectacular. Don't let anyone talk you into the stairs on a cruise day unless you are very fit.
Genuinely one of the most pleasant old towns in the Mediterranean. I walked the sea bastions for an hour, had a glass of Cannonau at a bar overlooking the water, and then had the best lobster I have ever eaten at a small place near the cathedral. Nothing complicated about the day at all.
I skipped the grotto entirely and just wandered the old town and the beach. The walls at sunset, the local wine, the sound of Catalan being spoken in a shop on a Sardinian island. It felt like nowhere else I had been. The cave is special but the town alone is worth the stop.
